Guidlines For Registration

1. Make cheques payable to Bear Lake Bible Camp.
2. Post-dated cheques are not accepted.
3. Cancellation Policy: One week before camp date, all but a $15 administration fee will be refunded. After 1-week deadline, fee is non-refundable.

6. Waivers and Conditions of Enrollment:
1. The Director reserves the right to dismiss a camper who, in his opinion, is a hazard to the safey and rights of others.
2. The parent or guardian(s) submitting this application are those having legal custody over the child. (If applicable, conditions of custody and a copy of the visitiation rights may be sent.)
3. Completion of the Medical Information section of the registration form.

If you are registering more than one child, 'Reset' the form after printing the first time, fill in the form a second time, and repeat the process as many times as necessary.
